The Heart and Vascular Center's Comprehensive Heart Failure program provides evaluation, management and treatment for patients with heart failure, a disease of the heart muscle which does not allow the heart to adequately support the rest of the body's functions.
Our multidisciplinary team is composed of specialists representing every aspect of cardiac care, including heart failure cardiologists and cardiac surgeons, as well as dedicated advanced practice providers, nurse coordinators, pharmacists, psychologists, transplant immunologists, dietitians, financial counselors, palliative care specialists and social workers. They are often among the first in the country to use new medicines, procedures and devices for every stage of heart failure.
Our goal is to provide each patient with a tailored treatment plan addressing a patient's physical, mental and emotional needs with nurse-led clinics, telemanagement, nutrition counseling and social services.
