On June 6, Yale New Haven Health officially launched its Home Hospital program, which provides high-acuity, hospital-level care to patients in their homes. The program currently serves patients who live near Yale New Haven and Bridgeport hospitals; it will expand to other areas. Using in-person visits and telehealth technology, physicians and other healthcare professionals care for patients with heart failure, pneumonia, chronic obstructive pulmonary disease, cellulitis, sepsis and other conditions.
