Marissa Antonio, RN, Diagnostic Radiology, Breast Imaging, was recognized for 200% accountability, attention to detail and practicing a questioning attitude. Two patients with the same last name had biopsy specimens collected the same time and date. Following protocol, Antonio cross-checked both specimens the day after collection and saw that one of the specimens was not in process. She spoke up for safety and escalated her concerns to the Laboratory, where an error was detected in the specimen receiving process. The appropriate corrections were quickly completed, the specimens processed and the results received correctly.
Erin Chesery, RN, Children’s Sedation Services, was recognized for attention to detail and practicing a questioning attitude. A Spanish-speaking patient presented to the Pediatric Diagnostic Radiology desk for an MRI with sedation. With an interpreter on the phone to help with the intake questions, Chesery noticed that the mother referred to the child as “she” although the patient was identified as male in Epic. She asked the mother to write the child's name and date of birth, which did not match the ID band. It was determined that this patient was instead scheduled for an ultrasound. Chesery helped the family get to the correct location and receive a new ID band.
