Andrea Barbieri, MD, Yale Medicine pathologist, was recognized for attention to detail, use of STAR (Stop, Think, Act, Review) and stopping the line. After reviewing pathology slides from an out-of-network gastroenterology center, Dr. Barbieri identified a tissue contamination in the sample. Further diagnostic testing confirmed her suspicion. Dr. Barbieri’s questioning attitude and escalation of concerns prevented unnecessary surgery.
Tabea Wilson, RN, PACU, was recognized for attention to detail and practicing a questioning attitude. While performing a pre-operative check, Wilson noticed a discrepancy in the surgery site location between the scheduling form, the consent and the surgeon’s note. One document listed the right side as the location; the others listed the left side. Wilson stopped the line and escalated her concerns to the surgical team. Imaging confirmed the correct site location, and forms were corrected.
