Brian Coutermarsh, Heart and Vascular Center interventional technologist, was recognized for attention to detail and speaking up for safety during a procedure. Coutermarsh was inspecting the equipment that had been implanted during the procedure, and noted that a marker used as an X-ray reference point was missing. Coutermarsh escalated his concerns to the attending interventional radiologist, who promptly performed an X-ray showing that the marker was in the patient’s right ventricle. Thanks to Coutermarsh’s use of CHAMP behaviors, a potentially critical complication was avoided.
David Tomasheski, RN, Utilization Review, was honored for his attention to detail, 200% accountability and escalating his concerns. While making rounds in the Emergency Department after a busy night, he noticed that a patient who had been brought in after a motor vehicle accident appeared very pale and confused. Tomasheski escalated his concerns to the ED attending physician, who immediately evaluated the patient and sent him to the operating room for a splenectomy.
