Inpatient
Alexander Hayden, physician assistant, Heart and Vascular Center cardiac surgery, was recognized for attention to detail, practicing a questioning attitude and stopping the line during a procedure. Hayden was observing a case and, because he had been involved in the pre-operative workup, noticed the team was preparing for a different procedure. He shared his concerns with the clinical team, who accepted his questioning attitude, and the procedure was changed.
Outpatient
Anne Marie Rivard, transplant dietitian, Transplant Clinic, was recognized for attention to detail. She noted a discrepancy between two documentations of a patient’s weight in the electronic medical record. Weights are particularly important and followed closely for transplant patients. Further investigation showed that patient weight was being entered into the record in two different ways: electronically and manually. For this patient, the difference in the two weights was clinically significant. The issue was forwarded to the clinical redesign team, which created one standard best practice for entering patient weights into the EMR.
