Congratulations to these Great Catch winners for modeling HRO safety behaviors and taking action to improve patient safety.
Colman Dobb, RN, and Emily Day, RN, Medicine (EP 6-7), were honored for practicing a questioning attitude and advocating for the safety of a patient with dementia who was awaiting placement in an extended-care facility. Following a family visit, the patient experienced a rapid physical decline. The family requested no interventions, and the chaplain was called. Concerned that the patient’s symptoms were consistent with an opioid overdose, Dobb and Day advocated for the administration of Narcan, which reversed the symptoms. Testing revealed the presence of oxycodone that the hospital team had not administered. Visitor restrictions were implemented and conservatorship was initiated.
Molly O’Lena, LCSW, was recognized for practicing a questioning attitude and attention to detail. A minor presented with suspected abuse to the Saint Raphael Campus Emergency Department with an adult. While the minor was being triaged, the adult’s behavior became suspicious. O’Lena escalated her concerns to the New Haven Police Department and the Department of Children and Families. The child was kept safe in the ED. It was determined that the child had been abducted from the parent’s home.
