These employees received PEACE (Patient Experience Award for Caring and Excellence) recognition for going above and beyond to meet patients’ and families’ needs.
A team was collectively honored for compassionate care of a patient with terminal cancer. After a discussion with the inpatient palliative care team, the patient expressed his wish to return to his home country before his death. The repatriation process and transfer of the patient’s care to an appropriate facility in the patient’s home country was a highly coordinated effort led by Jaime Habersang, RN; Urs Weber, MD; Erin Gombos, MD; Laura Morrison, MD; and Maribel Cabrera Martinez, MD. They worked with Care Management to prepare the patient for medical air transport out of YNHH. Staff and leadership from Nursing, Social Work, Pharmacy, Off Shift Clinical Executives and Post-Acute Care also assisted.
SLA 2, Protective Services, Religious Ministries and the Off Shift Clinical Executives were honored for patient-centered care. The patient, a retired first responder who served during the Sept. 11, 2001 attacks on the World Trade Center wanted to acknowledge the 20th anniversary of the attacks, despite being hospitalized away from her friends and extended family. Protective Services officers, SLA 2 staff, off shift clinical executives and an intern from Religious Ministries gathered in the patient’s room for a bedside prayer and tribute to commemorate the date.
