Great Catch winners help YNHH become a high reliability organization

Outpatient

Tracey Norton

Tracey Norton, CT technician, Diagnostic Imaging, was recognized for attention to detail and escalation of concerns while scanning two separate patients. While reviewing the images in real time, she noticed that each patient had concerning blood clots. She immediately escalated her concerns to the radiologist, who further examined the images, then contacted each patient’s ordering physician. Both patients were sent to the Emergency Department for evaluation and treatment.

A team from Women’s Services was recognized for collective use of CHAMP behaviors to ensure appropriate care of a postpartum patient. After a normal delivery, the patient displayed behavior that caused the nursing staff to question the safety of mother and infant. Staff requested further examination of the mother. Social Work and Psychiatry staff evaluated the patient for depression, while nursing staff escalated their concerns to the patient service manager, who brought the situation to the attention of the obstetrics physician and medical team. An MRI of the mother’s brain was ordered, and showed a frontal mass. The patient was immediately transferred to Neurosurgical services and underwent brain surgery.
